Health Benefits Will Now Be Offered To Distributors!



In an exceptional move for a relatively new company, SoZo will now offer a health benefits plan to Distributors. This is indicative of the strength of SoZo moving into the future. Final details currently are being negotiated with the intent of opening enrollment in the plan October 1, 2011, for coverage that will begin Jan. 1, 2012.

The program will offer three Limited Benefits Health Plans, the first with basic coverage, the second with slightly higher benefits and cost and the third with a very good program with a significantly higher cost. We MUST achieve at least 10% participation for the program to launch.

A Distributor may choose from four options: Distributor, Distributor plus spouse, Distributor plus children or Distributor plus family. Coverage will include

• Office, Urgent Care, and Outpatient Visits
• Diagnostics, X-Ray and Lab Benefits
• Emergency Room and Inpatient Hospital coverage
• Surgical and Outpatient surgical coverage
• Group Accident
• Patient Advocacy Services
• Dental and Vision coverage
• Outpatient Major Diagnostic Testing (only Tier II & III)
• Critical Illness Benefit (only Tier II & III)

Tier I Plan (Director & 200PV Autoship qualification)
Base coverage (estimated at $150)

Tier II Plan (Diamond Achiever & 200PV Autoship qualification)

SoZo contribution to premium expense

Tier III Plan (Diamond Executive & 200PV Autoship qualification)
Base coverage (estimated at $300)
Best coverage limits
Expanded coverage types
SoZo full coverage of premium expense

The Health Benefits Program does not qualify for COBRA should a Distributor leave SoZo. The Health Benefits Program cannot be used in conjunction with Medicare or Medicaid, and is not available outside the US.
